Any account of “what is special about the human brain” (Passingham 2008) must specify the neural basis of our unique ability to produce speech and delineate how these remarkable motor capabilities could have emerged in our hominin ancestors. Clinical data suggest that the basal ganglia provide a platform for the integration of primate-general mechanisms of acoustic communication with the faculty of articulate speech in humans. Furthermore, neurobiological and paleoanthropological data point at a two-stage model of the phylogenetic evolution of this (...) crucial prerequisite of spoken language: (i) monosynaptic refinement of the projections of motor cortex to the brainstem nuclei that steer laryngeal muscles, presumably, as part of a “phylogenetic trend” associated with increasing brain size during hominin evolution; (ii) subsequent vocal-laryngeal elaboration of cortico-basal ganglia circuitries, driven by human-specificFOXP2mutations.;>This concept implies vocal continuity of spoken language evolution at the motor level, elucidating the deep entrenchment of articulate speech into a “nonverbal matrix” (Ingold 1994), which is not accounted for by gestural-origin theories. Moreover, it provides a solution to the question for the adaptive value of the “first word” (Bickerton 2009) since even the earliest and most simple verbal utterances must have increased the versatility of vocal displays afforded by the preceding elaboration of monosynaptic corticobulbar tracts, giving rise to enhanced social cooperation and prestige. At the ontogenetic level, the proposed model assumes age-dependent interactions between the basal ganglia and their cortical targets, similar to vocal learning in some songbirds. In this view, the emergence of articulate speech builds on the “renaissance” of an ancient organizational principle and, hence, may represent an example of “evolutionary tinkering” (Jacob 1977). Mohr (Paul Siebeck), Tübingen 1939. - Das Werk, in dem Heinrich Rickert seine Lebensarbeit zusammenfassen wollte, ist sein "System der Philosophie." Der erste Band dieses Werkes enthält die "Allgemeine Grundlegung der Philosophie" (Tübingen, 1921). Der zweite Band sollte die "Philosophie des kontemplativen Lebens," der dritte die des "aktiven Lebens" behandeln. Der Umriß des Systems, der im ersten Bande mitunter vielleicht noch wie ein leeres Rahmen- und Fächerwerk wirken könnte, sollte nun (...) seine Brauchbarkeit zeigen. Er sollte angefüllt und bereichert werden durch sachhaltige Untersuchungen über die einzelnen Teilgebiete des philosophischen Systems. So war für den zweiten Band z. B. eine Wissenschaftslehre (Logik und Methodologie), eine Philosophie der Kunst (Ästhetik) und eine Behandlung der mystischen Religiosität geplant. Der dritte Band sollte vor allem die Sozialphilosophie enthalten; zur Sprache kommen sollte dabei zunächst das sittliche Leben in der "Gemeinschaft freier Personen" (Ethik), dann die über jede sittliche Verpflichtung noch hinausgehende Liebesgemeinschaft, wie sie z.B. in der Freundschaft und in der Ehe erlebt wird (Erotik), schließlich das religiöse Leben auf Grund persönlicher Gottesbegegnung. Druckfertig liegen für die inhaltliche Erfüllung und Ausgestaltung des Systems nur einige Aufsätze vor, die den zweiten Band eröffnen sollten: Rickert selbst hat noch den gesonderten Abdruck dieser Aufsätze geplant und ihre Anordnung genau festgelegt. Sein Plan wird im folgenden ausgeführt. Vorangestellt ist ein Zeitungsaufsatz über die Welt der Erfahrung, der in Kürze das Ziel der folgenden Untersuchungen teilweise sogar schon wörtlich vorwegnimmt. Da die Fehlurteile über Rickerts angeblich wirklichkeitsfremden Formalismus meistens nur darauf beruhen, dass man den Weg der Untersuchung mit ihrem Ziel, die Methode mit der Sache selbst verwechselt, so sei auf diesen kurzen Einleitungsaufsatz und dann namentlich auf den Abschnitt über Sprache und Antlitz der Körperwelt besonders hingewiesen. Die vorliegenden Aufsätze sollten also einen Übergang herstellen von den noch mehr formalen Ausführungen der Allgemeinen Grundlegung zu den dann immer sachhaltiger werdenden Einzeluntersuchungen über den inhaltlichen Reichtum des kontemplativen und des aktiven Lebens.[August Faust; Breslau 1939; Auszug aus dem Vorwort des Herausgebers]. Band 9 der Schriftenreihe Beitrage zur Angewandten Ethik versammelt Arbeiten zu aktuellen Problemen sowie zu Grundlagenfragen der Medizinischen Ethik. Es handelt sich dabei zum Teil um Beitrage zu wissenschaftlichen Symposien, die das Interuniversitare Institut fur Angewandte Ethik in den Jahren 2008-2011 veranstaltet hat, und zum anderen Teil um Forschungsarbeiten, die im Rahmen von Projekten dieses Instituts entstanden sind. Zu den aktuellen Problemen der Medizinischen Ethik, die in den einzelnen Beitragen angeschnitten werden, gehoren Fragen der Stammzellenforschung und der Praimplantationsdiagnostik ebenso wie (...) Fragen, die sich im Zusammenhang mit den bei der In-Vitro-Fertilisation entstehenden 'uberzahligen' Embryonen ergeben, sowie Fragen bezuglich der neuen Patientenverfugungen - also durchwegs Fragen von hochster Aktualitat und Brisanz. Background: Compassion fatigue is recognized as impacting the health and effectiveness of healthcare providers, and consequently, patient care. Compassion fatigue is distinct from “burnout.” Reliable measurement tools, such as the Professional Quality of Life scale, have been developed to measure the prevalence, and predict risk of compassion fatigue. This study reviews the prevalence of compassion fatigue among healthcare practitioners, and relationships to demographic variables. Methods: A systematic review was conducted using key words in MEDLINE, PubMed, and Ovid databases. Data were (...) extracted from a total of 71 articles meeting inclusion criteria, from studies measuring compassion fatigue in healthcare providers using a validated instrument. Quantitative and qualitative data were extracted and compiled by three independent reviewers into an evidence table that included basic study characteristics, study strength and quality determination, measurements of compassion fatigue, and general findings. Meta-analysis, where data allowed, was stratified by Professional Quality of Life version, heterogeneity was quantified, and pooled means were reported with 95% confidence interval. A table of major study characteristics and results was created. Ethical consideration: This paper contains no primary data obtained directly from research participants. Data obtained from previously published resources have been acknowledged within references. Psychological distress, particularly compassion fatigue, can be insidious, no health profession is immune, and may significantly impact the ability to provide care. Results: A total of 71 studies were included. Compassion fatigue was reported across all practitioner groups studied. Relationships to most demographic variables such as years of experience and specialty were either not statistically significant or unclear. Variability in reporting of Professional Quality of Life results was found. Interpretation: Compassion fatigue exists across diverse practitioner groups. Prevalence is highly variable, and its relationship with demographic, personal, and/or professional variables is inconsistent. Questions are raised about how to mitigate compassion fatigue. (shrink)

Health-related Quality of Life measures have recently been attacked from two directions, both of which criticize the preference-based method of evaluating health states they typically incorporate. One attack, based on work by Daniel Kahneman and others, argues that ‘experience’ is a better basis for evaluation. The other, inspired by Amartya Sen, argues that ‘capability’ should be the guiding concept. In addition, opinion differs as to whether health evaluation measures are best derived from consultations with the general public, with patients, or (...) with health professionals. And there is disagreement about whether these opinions should be solicited individually and aggregated, or derived instead from a process of collective deliberation. These distinctions yield a wide variety of possible approaches, with potentially differing policy implications. We consider some areas of disagreement between some of these approaches. We show that many of the perspectives seem to capture something important, such that it may be a mistake to reject any of them. Instead we suggest that some of the existing ‘instruments’ designed to measure HR QoLs may in fact successfully already combine these attributes, and with further refinement such instruments may be able to provide a reasonable reconciliation between the perspectives. In this article, we propose the Fair Priority Model for COVID-19 vaccine distribution, and emphasize three fundamental values we believe should be considered when distributing a COVID-19 vaccine among countries: Benefiting people and limiting harm, prioritizing the disadvantaged, and equal moral concern for all individuals. The Priority Model addresses these values by focusing on mitigating three types of harms caused by COVID-19: death and permanent organ damage, indirect health consequences, such as health care system strain and stress, as well as (...) economic destruction. It proposes proceeding in three phases: the first addresses premature death, the second long-term health issues and economic harms, and the third aims to contain viral transmission fully and restore pre-pandemic activity. -/- To those who may deem an ethical framework irrelevant because of the belief that many countries will pursue "vaccine nationalism," we argue such a framework still has broad relevance. Reasonable national partiality would permit countries to focus on vaccine distribution within their borders up until the rate of transmission is below 1, at which point there would not be sufficient vaccine-preventable harm to justify retaining a vaccine. When a government reaches the limit of national partiality, it should release vaccines for other countries. -/- We also argue against two other recent proposals. Distributing a vaccine proportional to a country's population mistakenly assumes that equality requires treating differently situated countries identically. Prioritizing countries according to the number of front-line health care workers, the proportion of the population over 65, and the number of people with comorbidities within each country may exacerbate disadvantage and end up giving the vaccine in large part to wealthy nations. All parties involved in researching, developing, manufacturing, and distributing COVID-19 vaccines need guidance on their ethical obligations. We focus on pharmaceutical companies' obligations because their capacities to research, develop, manufacture, and distribute vaccines make them uniquely placed for stemming the pandemic. We argue that an ethical approach to COVID-19 vaccine production and distribution should satisfy four uncontroversial principles: optimising vaccine production, including development, testing, and manufacturing; fair distribution; sustainability; and accountability. All parties' obligations should be coordinated and mutually consistent. For (...) instance, companies should not be obligated to provide host countries with additional booster shots at the expense of fulfilling bilateral contracts with countries in which there are surges. Finally, any satisfactory approach should include mechanisms for assurance that all parties are honouring their obligations. This assurance enables countries, pharmaceutical companies, global organisations, and others to verify compliance with the chosen approach and protect ethically compliant stakeholders from being unfairly exploited by unethical behaviour of others. From the perspective of biological cybernetics, “real world” robots have no fundamental advantage over computer simulations when used as models for biological behavior. They can even weaken biological relevance. From an engineering point of view, however, robots can benefit from solutions found in biological systems. We emphasize the importance of this distinction and give examples for artificial systems based on insect biology.
